We currently have only one physician taking New Patients.  However, we are not adding any more patients to our waitlist at the moment.  We hope to open up more spots in the near future.  Please call our office to inquire about the wait time.

What is a Family Health Organization (FHO)?  

Our physician team at Mount Dennis Weston Health Centre is part of the Humber River Family Health Organization, FHO.  A FHO is a group of family physicians who are working together to give you and your family better access to quality primary services.

What is a Family Health Team (FHT)? 

Family Health Team’s (FHTs) are an approach to primary health care that brings together different health care providers to co-ordinate the best patient care possible for you – the patient. Our team works in collaboration with the Humber River Family Health Team (https://www.hrfht.com).  The HRFHT consists of Nurse Practitioners, Social Workers, Nurses, Dieticians, Chiropodists and a Pharmacist.

I already have a Doctor at your clinic but would like to request to have a different one?

Unfortunately, we do not accept requests for transfer to a different physician at our clinic.  We currently have a long wait list for patients waiting to join our practice who do not have family physicians at all.
