We currently have only three Physicians taking New Patients.  We currently have a wait list for patients waiting to join our practice who do not have Family Physicians at all.  Please call our office to inquire about the wait time.

What is a Family Health Organization (FHO)?  

Our physician team at Mount Dennis Weston Health Centre is part of the Humber River Family Health Organization, FHO.  A FHO is a group of family physicians who are working together to give you and your family better access to quality primary services.

What is a Family Health Team (FHT)? 

Family Health Team’s (FHTs) are an approach to primary health care that brings together different health care providers to co-ordinate the best patient care possible for you – the patient. Our team works in collaboration with the Humber River Family Health Team (https://www.hrfht.com).  The HRFHT consists of Nurse Practitioners, Social Workers, Nurses, Dieticians, Chiropodists and a Pharmacist.

I already have a Doctor at your clinic but would like to request to have a different one?

Unfortunately, we do not accept requests for transfer to a different physician within our clinic.  We currently have a long wait list for patients waiting to join our practice who do not have family physicians at all.
