Our clinic is a proud partner of the Humber River Family Health Team.

 

Family Health Teams provide Interprofessional Health Provider (IHPs) services by working with family physicians, nurse practitioners, registered nurses and a range of other health care professionals.

​

As a clinic we provide patients access to these professionals with a streamlined referrals process - and we share office space too for ease of access. The HRFHT operates at several locations to ensure access for rostered patients.

 

To support community care as a whole, the HRHFT develops ongoing community-based programs that emphasize health promotion, illness prevention and early detection and diagnosis.

 

The HRHTF can connect you to other parts of the health care system such as acute care, long-term care, public health, mental health, addictions and community programs and services.

• Monday, Tuesday, Wednesday: 9-5pm

• Thursday: 12-8pm

• Friday: 8-4pm

• One Saturday per month: 9-3pm.
