Referral Requests

To request a referral to a specialist or specialized services, please schedule an appointment with the clinic.

Before we can send the referral, we usually need to ask a number of questions and do a physical exam, so that we can adequately assess and describe the issue or condition.

We will try to find the best specialist for your specific issue or service need. We try to balance wait time, location, personal experience and specialist expertise.

Ensuring that the referral is successful is a shared responsibility. Please reach out to us if you have not been contacted more than two to three weeks after the referral was initiated.

Contact our referrals coordinator at (416) 763-1171 extension 212. We will do our best to provide an update or have your physician review the need to find an alternate specialist.  We triage referral requests to the most urgent first.

Referral appointments can take time to reschedule. Please do your best not to cancel or miss your appointment time.

Email and Voicemail Alert

Many specialists will email your appointment details to the email on file. Be sure to check your Spam or Junk folders regularly to ensure you don't miss their communication.

Similarly, please check your voicemail to ensure you don't miss information left about your appointment.​
