Sick Notes and Forms

OHIP does not pay for sick notes, absentee notes, or some work-related forms.

There is a fee for these items. However if you have paid your physician for the Annual Block Fee some of these services are covered.

​

Some places of employment reimburse their employees for the cost of sick notes; please check with your employer.

 

If you need a note from us, please inform us at the time of booking, so that we can allocate time to complete it as required.

Please ensure you complete any sections of forms that are for patient information prior to bringing your forms to the office. Your physician can provide information and signature on completed forms.

Fees can be paid online, or in person.
